The Age-friendly Health Systems movement was launched in 2017 to improve health outcomes and reduce preventable harm among older adults. The movement is centered around the 4Ms framework: What Matters, Medications, Mentation, and Mobility. These four pillars provide a way to organize and approach care of older adults, with a focus on an individual’s preferences and care goals. This seminar will explain the Age-friendly Health Systems movement, the importance of advocating for this approach to care, and discuss how the 4Ms can be applied in healthcare settings and beyond. The American Diabetes Association (ADA) has incorporated the 4M model into their 2025 guidelines recognizing its importance in our aging population. Older adults with diabetes face unique challenges due to the complex interplay of chronic conditions, cognitive changes, and functional limitations. The 4Ms framework can be assimilated across industries to provide continuity of care for our clients with diabetes. Come learn how you can incorporate this model into your practice!
